Why do we have to fill out these forms?

These forms are filled out by every 4-H'er, for every 4-H event through Georgia. These forms are to ensure the safety of our 4-H'ers.

The 4-H Code of Conduct should be read and signed by both parent and 4-H'er. The Code of Conduct states the guidelines for disciplinary actions that could be taken if the need arise. This is to ensure that all 4-H'ers and 4-H parents understand the rules and policies of 4-H events.

Medical Information and Release Form

The 4-H Medical Form should be read and signed. There are 2 sides to this form. The Medical Form is primarily for emergencies, but this form also gives on-site nurses and extension personnel parental permission to administer medications to a 4-H'er in the absence of a parent or legal guardian. Permission to use over-the-counter medications, bandages and external ointments for scraps must be stated on the reverse side of the medical form.
